The LPN/RN provides essential nursing care and health support services to residents in a structured reentry setting. This role focuses on promoting physical and mental well-being, supporting individuals transitioning back into the community, and ensuring compliance with all medical and regulatory requirements. The ideal candidate is compassionate, professional, and able to maintain strong boundaries while working with a diverse population, including individuals with criminal backgrounds.
Job Duties
– Administer medications and monitor residents for side effects or adverse reactions
– Conduct health assessments, screenings, and ongoing monitoring of residents
– Maintain accurate and confidential medical records in compliance with HIPAA
regulations
– Coordinate care with physicians, mental health providers, case managers, and staff
– Respond to medical concerns, emergencies, and incidents as needed
– Provide basic medical care, first aid, and health education to residents
– Monitor and manage chronic health conditions and medication compliance
– Assist with intake medical evaluations and discharge planning
– Ensure proper documentation of all medical interactions and treatments
– Maintain strict adherence to policies, procedures, and regulatory standards
– Support a safe, structured, and respectful environment for residents and staff
Environment and Population Considerations
– Work with individuals transitioning from incarceration into the community
– Maintain professional boundaries while building trust and rapport
– Be aware of potential behavioral, mental health, and substance abuse challenges
– Support rehabilitation efforts while ensuring safety and compliance
– Work collaboratively with security and program staff when necessary
Job Requirements
– Active LPN or RN license (in good standing)
– Minimum 1+ year of nursing experience (correctional, behavioral health, or community
health preferred)
– Strong knowledge of medication administration and patient care practices
– Ability to handle high-stress or sensitive situations with professionalism
– Excellent documentation and organizational skills
– Strong communication and interpersonal skills
– Ability to maintain confidentiality and ethical standards
– Reliable attendance and strong sense of accountability
Preferred Qualifications
– Experience in correctional facilities, halfway houses, or behavioral health settings
– Knowledge of substance abuse disorders and recovery support
– CPR/First Aid certification (or willingness to obtain)
– Experience with electronic medical records (EMR systems)
– Crisis intervention or de-escalation training
Safety & Work Environment
– Work performed in a secure residential facility
– Interaction with individuals who may have criminal backgrounds
– Must remain alert, aware, and adhere to all safety protocols
– Possible exposure to challenging behaviors or emergency situations
– Must follow all healthcare regulations, safety standards, and internal policies
